If you did not return a consent form last March, you will receive an additional email with an electronic consent form. This must be returned to me before the end of Tuesday (15th) next week in order for the correct number of vaccines to be ordered.

Please can you ensure that your child has eaten breakfast on the morning of Tuesday 5th January and has a bottle of water with them.
